1. Consent and processing your data lawfully

By filling in any forms on this website, you are giving your consent for Leora Sharp, to hold your personal information, for as long as deemed necessary. The information which you supply on the forms will be emailed directly to me and not stored on this website. I use your information to give you the best possible service and for insurance purposes.

The lawful basis for processing your data is; consent, contractual, legitimate interest, legal obligation. You can read more about your data protection rights here.

2. Storing and retaining your information

When you come for your first appointment I will not take any contact information, unless we agree this is necessary. I will note down what you tell me about why you have come to see me and some of your body and possibly your emotional history. Only after you return to me a second time, will I ask you for contact details and you have a choice in how you would be contacted and for what purpose.

Your case notes are hand written and stored in a fireproof safe. When I carry them with me to my place of work, keep them safe at all times. Any contact details and email communication is cloud based and encrypted, provided by a reputable company. My computers and mobile phone are both password protected and encrypted.

If you have enquired about massage and not followed through with an appointment, I will delete any information and emails after 1 year.

If you become a client, and we do not have contact for 3 years, I will delete your contact details and emails. I cannot delete your details off the Neal’s Yard booking system, you will need to contact them to do that. Please see No. 5 below.

3. Accessing, updating or deleting your information

Your records are accessed by myself, for clinical purposes. You have the right to request a copy of your records at any time. Please use this form to make any request with regards to your information. This can include requesting to see the personal information I hold about you, updating or requesting deletion of information. If requesting to see information, this will be emailed to you.

As mentioned above, I am legally bound by my insurance company to hold your case notes for 7 years. I can therefore only shred the case notes 7 years after your last appointment. In exceptional circumstances, we can contact my insurance company for advice.

4. Sharing your information

With your permission, I need to share your preferred contact details with Neal’s Yard, to make a booking, see 5 below. Your name might also be seen by my bookkeeper and accountant.

The rest of your information will be kept strictly confidential unless there are legal grounds for me to disclose something which you have told me to a third party. I will always inform you if I feel that disclosure is necessary and seek your permission and co-operation first. I might also need to contact your doctor for medical reasons, and for this purpose, I will also seek your permission.

Should there be a need for me to seek guidance and clarity about your case from my supervisor, I will mention only the necessary case details, on an anonymous basis, in our confidential session. This is routine for health and wellbeing providers.

5. Neal’s Yard onilne booking software

Neal’s Yard, where I practice, use an online booking system which have their own privacy policies. I recommend that you read their privacy policies so you can understand the manner in which your personal information will be handled.

In particular, remember that certain providers may be located in or have facilities that are located in a different jurisdiction than either you or us. So if you elect to proceed with a transaction that involves the services of a third-party service provider, then your information may become subject to the laws of the jurisdiction(s) in which that service provider or its facilities are located.

6. Paying by card, over the phone, for your first appointment

Neal’s Yard, where I practice, require that initial consultation appointments are paid for by card, over the phone.

7. Email marketing

I do not use a 3rd party email marketing programme and don’t send out newsletter as such. With your permission and communication preference, I would like to send you emails directly from my inbox or phone (SMS), about my practice/special offers and post consultation check in, client specific wellbeing tips/advice, relevant event information. You have the option to positively opt in to receive these updates. You can unsubscribe at any time or please email me to ask me leora@freemyself.net

9. Complaints and reporting of data breach

If you are concerned there has been a data breach, or you are unhappy with how I am handling your data, please raise your concerns with me first, so that we can seek a resolution. If you are still not satisfied, then you have the right to complain to the Information Commissioner’s Office (ICO).

This website nor its owners will ever ask for personal or sensitive information through social media platforms and encourage users wishing to discuss sensitive details to contact them through primary communication channels such as by telephone or email.
